You will be notified by the Financiall Aid Office IF you are selected for verification. You will be directed as to what specific information must be returned to our office. If selected for verification, you do not need to submit all of the forms listed below.
Tax Transcript Information
In lieu of requesting a tax return transcript from the IRS, you/your parent have the option to use the IRS Data Retrieval Tool in FAFSA on the Web, which will automatically transfer your/your parent’s information from the IRS to your FAFSA once you have filed a 2012 tax return. This tool will be available two weeks after you file your taxes electronically or eight weeks after you submit your paper tax return.
- Log into the FAFSA website with student SS# and PIN#.
- Choose “make FAFSA corrections.”
- Hit “next” on the bottom of the screens until you get to the page titled “Parent Financial Information.”
- Answer question about tax filing status. Reminder: you will need the parent PIN#.
- If you are able to choose “Already Filed” AND “none of the above” follow screen prompts to Link DATA to IRS.
- If you unable to use the IRS data retrieval tool, you MUST request a tax transcript, please see the reverse side for those instructions.
- Enter PIN # and click “Link to IRS.”
- Follow the same steps for the student section. You will need the student’s PIN #.
- When complete, follow screen prompts until you get to the “sign and submit” page. You will need both the student and parent PIN # again.
- You will know your FAFSA was submitted when you get the following screen information: “Congratulations, STUDENT NAME, your FAFSA was successfully submitted….”
- The Financial Aid office will receive your corrected FAFSA in 3-7 business days, at which time they will review your financial aid file and complete the verification process.
If you are unable to use the IRS Data Retrieval Tool, you will need to obtain an IRS tax return transcript for you and/or your parent, go to www.IRS.gov and click on the “Order a Return or Account Transcript” link, or call 1-800-908-9946. Make sure to request the “IRS tax return transcript” and
not the “IRS tax account transcript.” You will need the Social Security Number, date of birth, and the address on file with the IRS (normally this will be the address used when the 2012 IRS tax return was filed) for the tax filer. It takes up to two weeks for the IRS income information to be available for electronic tax return filers, and up to eight weeks for paper IRS tax return filers. The tax transcript will be sent to you, at which time you need to forward a copy of it to the Financial Aid Office.
2012-2013 Forms
These forms apply to the 2012-2013 academic year, which began in August of 2012. Your 2012-2013 FAFSA was filed using 2011 income tax information.
2012–2013 Verification of Household Size Worksheet: You may need to submit this worksheet if you are selected for verification for the 2012–2013 academic year, in which case, you will be notified by the Financial Aid Office.
2012–2013 Child Support Paid Form: You may need to submit this worksheet if you are selected for verification for the 2012–2013 academic year, in which case, you will be notified by the Financial Aid Office.
2012–2013 Student Non-Filer Form: You may need to submit this worksheet if you are selected for verification for the 2012–2013 academic year, in which case, you will be notified by the Financial Aid Office.
2012–2013 Parent Non-Filer Form: You may need to submit this worksheet if you are selected for verification for the 2012–2013 academic year, in which case, you will be notified by the Financial Aid Office.
2012–2013 Spouse Non-Filer Form: You may need to submit this worksheet if you are selected for verification for the 2012–2013 academic year, in which case, you will be notified by the Financial Aid Office.
2013-2014 Forms
These forms apply to the 2013-2014 academic year, which begins in August of 2013. Your 2013-2014 FAFSA was filed using 2012 income tax information.
2013-2014 Verification of Household Size Worksheet: You may need to submit this worksheet if you are selected for verification for the 2013–2014 academic year, in which case, you will be notified by the Financial Aid Office.
2013-2014 Child Support Paid Form: You may need to submit this worksheet if you are selected for verification for the 2013–2014 academic year, in which case, you will be notified by the Financial Aid Office.
2013-2014 Student Non-Filer Form: You may need to submit this worksheet if you are selected for verification for the 2013–2014 academic year, in which case, you will be notified by the Financial Aid Office.
2013-2014 Parent Non-Filer Form: You may need to submit this worksheet if you are selected for verification for the 2013–2014 academic year, in which case, you will be notified by the Financial Aid Office.
2013-2014 Spouse Non-Filer Form: You may need to submit this worksheet if you are selected for verification for the 2013–2014 academic year, in which case, you will be notified by the Financial Aid Office.
2013-2014 Homeless Renewal Form: You may need to submit this form is you indicated that you were homeless when you completed the 2013-2014 FAFSA. You will be notified by the Financial Aid Office if this document is required.
Church Matching (PIE) Grant Application: Churches may donate funds in your name, and North Park will match these funds up to $500 per year. This application must be completed by your church annually.
FERPA Release Form: Once you enroll at North Park, your educational records will be protected under the Family Educational Rights and Privacy Act (FERPA). Release of your educational record is not allowed without written consent. This includes parents and family members. If you wish to have information released to a parent, guardian, or third party, please complete this form and return it to our office. Examples of educational records include, but are not limited to, your telephone listing, grades, billing/financial aid, discipline, and/or medical records.
Satisfactory Academic Progress Appeal Form: This form must be completed if you do not meet the minimum standards of Satisfactory Academic Progress, in which case, you will be notified by the Financial Aid Office.
Appeal Forms: If you need to provide additional information to the Financial Aid Office, such as unusual family expenses, lost income, or other factors that might reduce your and your family's ability to meet your Estimated Family Income (EFC), please contact Carolyn Lach by phone at (773) 244-5506 or email.
