Request a Transcript

Your academic record is part of your permanent education record and is therefore protected under the Family Education Rights and Privacy Act of 1974. As such, University staff will not release any protected information (e.g., grade information, personal data) to a third party without your express written consent (by letter, fax, or transcript request card). For questions about the types of information not protected by FERPA, please consult the North Park Student Handbook or contact the Office of Student Administrative Services.

Transcripts issued will be embossed and individually sealed as official in a University marked envelope.  It is institutional policy not to fax transcripts or grade reports to anyone for any reason because the integrity of transmitting student information in this way cannot be guaranteed.

The University provides one copy of your official transcript free of charge.  All other transcript requests are $5.00 per copy ($10.00 per copy for guaranteed service within 24 hours.) We accept Visa, MasterCard, checks, money orders, or cash as payment. If you are paying by credit card, please include your credit card type, credit card number, and expiration date at the bottom of the request.  Transcripts cannot be processed until payment is received.

To request a transcript, you will need to complete a Transcript Request Card and mail it to the address provided below or fax it to 773-244-4954.

If you are unable to print out the Transcript Request card, you will need to provide the following information in order to process your request:

1. Your name (and maiden name, if applicable)
2. Your Social Security number
3. Your date of birth
4. Your address
5. Your phone number
6. List dates of attendance or if you are a current student
(e.g., undergraduate, MBA, Seminary...), and the date of graduation (if applicable)
7. Number of copies
8. Provide exact name and address where the transcript will be sent
9. Your signature
