Responsibility of Student
Student will self-identify him/herself to the Office of Student Enrichment Services and Support (traditional undergraduate students) as a student with a disability and in need of accommodations. All other students will need to notify their Program Director. The Office of Student Enrichment Services and Support and/or the Program Director must be notified at least two weeks in advance in order to pursue needed accommodations or services. Depending on the nature of the program, service or activity, accommodations may take longer or be able to be coordinated in less time.
Student must provide recent (within the past three years) documentation of diagnosed disability. Documentation must be on the letterhead of a professional or agency, must state the specific disability, detail the nature of the disability, and include recommendations for accommodations. See

Student must meet with a member of the Office of Student Enrichment Services and Support and/or the Program Director each semester so that their case may be reviewed and reasonable accommodations determined. Reasonable accommodations are determined on a case by case basis.
Student will adhere to course content, academic standards, policies and procedures set forth by the professor, department and/or university, as all students are required to follow.
Student will meet with his/her individual professor to discuss their learning needs and the specific accommodations needed during the course.
Student must notify the Office of Student Enrichment Services and Support and/or the Program Director if the accommodations and services are not effective.
