Lake County Scholarship Application

Use this form to apply for the Lake County Scholarship. The application deadline is August 15, 2012.

Eligibility guidelines:

• Newly admitted to the School of Adult Learning for Fall 2012
• A resident of Lake County, Ill.
• Transferring at least 30 semester hours, with a minimum 3.0 transfer GPA

In addition to completing the form below, you must do the following to complete your application for this scholarship:

• Provide official transcript (mailed separately, directly to North Park University)
• Provide a letter of recommendation from a teacher, clergy member, employer, or volunteer organization (sent separately to Leslie Bertholdt)
