Syllabus Statement

Faculty and course instructors are strongly encouraged to include one of the following statements in each of their course syllabi. The statement makes it clear that if a student has a disability it is the student's responsibility to seek accommodations and directs the student to the appropriate office. You only need to include the phone number that applies to your specific program.

Traditional Undergraduate Students

Students with disabilities who believe that they may need accommodations in this class are encouraged to contact the Student Support Specialist by email or by phone at (773) 244-5737, or stop by the office located on the 2nd floor of the Student Services Building. Please do so as soon as possible to better ensure that such accommodations are implemented in a timely manner. If needed, appointments can be relocated.
