Use this form only if you have already been approved for a Federal Parent PLUS Loan and need to modify your existing loan. If you have not borrowed a Parent PLUS Loan this academic year, apply online at studentaid.gov using your FSA ID instead.

Loan change considerations:

• Your maximum loan is the cost of attendance minus other financial aid received. Consider borrowing for the full academic year upfront.
• Loan amounts are divided equally between fall, spring, and summer semesters.
• Loans disbursed after July 1, 2024, have a 4.228% origination fee, reducing the actual amount posted to your account. (Example: Borrowing $8,000 results in $7,662 posted, but you repay $8,000.)
• All fields on this form are required.

• Church Matching (PIE) grant application: North Park matches church donations up to $500 per year. The church must submit this form annually. (A separate Seminary Church Matching Grant application is available here.)
• FERPA release form: To grant parents or others access to your educational records (grades, billing, financial aid, etc.), complete this form. Without written consent, records cannot be released under FERPA.
• Satisfactory academic progress appeal form: Required if you do not meet Satisfactory Academic Progress standards. The Financial Aid Office will notify you if needed.
• Credit balance hold forms:
  • For students: Authorizes excess aid to remain on your account instead of being refunded.
  • For parents: Allows a parent borrower to keep the excess balance on the student’s account.
• Return credit balance form: If a credit balance occurs due to a change in enrollment, you can reduce your loan by the excess amount instead of receiving a refund, lowering what you repay.
• Appeal forms: If you experience financial hardships (lost income, high expenses, etc.), submit the Appeal Intent Form to financialaid@northpark.edu. Additional documents may be required.
