If selected for verification, you may need to submit one or more of the forms below. The Financial Aid Office will notify you if any are required. Use this form only if you have already been approved for a Federal Parent PLUS Loan and need to modify your existing loan. If you have not borrowed a Parent PLUS Loan this academic year, apply online at studentaid.gov using your FSA ID instead. Loan change considerations: Satisfactory academic progress appeal form: Required if you do not meet Satisfactory Academic Progress standards. The Financial Aid Office will notify you if needed.
