Chicago Travel Order

The Emergency Travel Order issued by the Chicago Department of Public Health (CDPH) now places states into one of two categories — orange and yellow — based on the incidence of COVID-19 in each state compared to conditions in Chicago. There are different requirements depending on what “color” state or other country you are coming from.

IMPORTANT EXEMPTION: Students who provide proof of a positive Covid-19 test result taken within 90 days before arrival in Chicago — and who have fully recovered — are presumed to be immune and are exempt from the Chicago Travel Order. Those who were tested through the University and Swedish Hospital do not need to provide proof.

Requirements:

ORANGE States

People who have been in an ORANGE STATE within 10 days of arrival in Chicago must do ONE of the following:

  • Sign the Attestation of Travel and Quarantine Form within 72-hours of returning to campus confirming that they quarantined for the 10-days prior to their arrival in Chicago.

OR

  • Submit a negative or pending Covid-19 test result, via one of the following methods (an antibody test does not qualify):
    • Provide proof of a negative test result taken 72-hours or less before arriving in Chicago.
    • Provide proof of test with the result pending, taken 72-hours or less before arriving in Chicago, and quarantine until results are available.
    • Undergo Covid-19 testing upon arrival in Chicago and quarantine until results are available

YELLOW States

There are no requirements for people arriving in Chicago from Yellow States.

International Travel

People who have been in a COUNTRY OUTSIDE THE U.S. within 10 days of arrival in Chicago must do BOTH of the following:

  • Sign the Attestation of Travel and Quarantine Form within 72-hours of returning to campus confirming that they quarantined for the 10-days prior to their arrival in Chicago.

AND

  • Submit a negative or pending Covid-19 test result, via one of the following methods (an antibody test does not qualify):
    • Provide proof of a negative test result taken 72-hours or less before arriving in Chicago.
    • Provide proof of test with the result pending, taken 72-hours or less before arriving in Chicago, and quarantine until results are available.
    • Undergo Covid-19 testing upon arrival in Chicago and quarantine until results are available

Attestation Form

Please complete the attestation form within 72-hours of returning to campus. This form is to document adherence to the travel order, including quarantine and test results when applicable.

Guidelines for Parents/Family Members

Parents who accompany students to campus to help with move-in should have documentation of a negative result from a diagnostic test taken within 72 hours before arrival in Chicago (antibody tests not acceptable). Parents from high-incidence states visiting Chicago for a reason other than to assist their child with move-in must follow the same guidelines as anyone else traveling inbound to Chicago from Orange states under the new order.
